  AoA’s previous Strategic Action Plan provided a framework for modernizing the role of the Aging Services Network in long-term care. This modernization strategy was designed to strengthen the Network’s role in advancing systemic changes in long-term care at the state and community-level. This Strategic Action Plan for 2007-2012 continues the focus on modernizing the Aging Services Network’s role in long-term care, and gives particular attention to implementing the new provisions contained in the Older Americans Act Amendments of 2006.

  This plan reflects the Agency of Human Services’ vision that Vermonters are healthy, safe and achieve their greatest potential for well-being and personal independence in healthy, safe and supportive communities as well as the Department’s commitment to increasing investments in community-based systems of long-term services and supports, to increasing consumer flexibility and choice and ensuring that older adults and individuals with disabilities have easy access to well-coordinated and effective services that promote healthy aging and independent living.
