Quality Management Grant

In September of 2004, the Department of Aging and Independent Living received a 3-year $499,709 grant to develop a comprehensive quality management system across all home and community-based waiver services within the Division of Disability and Aging Services (DDAS). The grant will be used to: Develop a Quality Management Plan addressing all Medicaid wavier services programs.

• Include consumers, their families and community members as active participants in Vermont's quality management activities.

• Develop and implement quality management activities to improve supports and services to Vermont's aging citizens and those with disabilities.

• Develop a technology-based system to manage and analyze critical incidents.

• Develop an ongoing system of technical assistance to all providers of services across age and disability and provide training to service recipients and relevant staff.
