What, Who, How & Why?
What is it?
MyCare Vermont is a planning grant funded with Federal dollars to integrate the assortment of health care an individual may need.
Who will be eligible?
Vermonters who are elderly, frail, at-risk or chronically ill, and adults with physical disabilities.
How will it work and how is it different?
Choices for Care:
- Provides person-centered care for individuals 18 years and older.
- Changes delivery of long-term care services.
- Provides equal access to nursing facility or home and community based services.
PACE:
- Provides person-centered care for individuals 55 years and older.
- Integrates primary/acute and long-term care services through an interdisciplinary team.
- Operates at a PACE center.
MyCare Vermont will:
- Provide person-centered care for individuals over the age of 18.
- Integrate primary/acute and long-term care services through an interdisciplinary team.
- Operate through community networks.
Why will this change be good for Vermonters?
- Person-centered care is at the heart of an interdisciplinary team.
- Lack of integration makes the current system inefficient.
- An integrated system generates better outcomes.
