System of Care Plan

  • Vermont State System of Care Plan for Developmental Services - FY1999 - Plan Period: July 1, 1998 - June 30, 2001

  • The Vermont System of Care Plan and Annual Report for Services to Adults and Children with Mental Retardation and Their Families 1996
    This report describes services supported by the Division of Developmental Services to adults and children with developmental disabilities and their families in FY 1995. The first section of the report has a special introduction that focuses on the achievements of the Vermont system of services and supports. There is also a brief overview of the work being done on the System of Care Plan. The second part of the report describes the status of these services, current pressures facing the delivery system, outcomes of services and future directions and goals. Policies

  • Background Check Policy (Revised: April 2003)
    Background checks are required for all workers who are paid with state or federal funds administered by the Division of Developmental Services (DDS), and who provide care to a person with a developmental disability; or handle funds on behalf of a person with a developmental disability.

  • Critical Health Care Decisions Policy (November 1996)
    A Division of Disability and Aging Services (DDAS) policy that outlines the role of the public guardian when needing to make critical health care decision for adults with developmental disabilities for whom they are guardian. It includes an outline of the role of the Ethics Committee.
